Paciente lactante de 2 años de edad asiste a consulta con el especialista de odontopediatría por presentar unión de la corona del diente 51 y 52, asociada a una anomalía de forma en los dientes, su motivo de consulta era debido a su frenillo lingual grueso, por lo que al examen intrabucal la Odontopediatra pudo apreciar que presenta fusion de ambas coronas del diente através del esmalte dental, por lo que al paciente se le mandaron hacer radiografías periapicales para determinar si esta anomalía es sólo a nivel del esmalte o si los dientes permanentes también se encuentran unidos, el examen radiográfico nos permitirá determinar un diagnóstico y plan de tratamiento adecuado para este pequeño.

Cuando hablamos de anomalía, existe una incidencia durante la vida intrauterina, esta no es más que una anomalía dentaria que se da por medio de la unión embriologica de dos germenes dentarios por medio del esmalte y dentina, normalmente estos dientes que se encuentran fusionados poseen cada uno su cámara pulpar, por lo que que no compartir la pulpa pero puede presentarse en algunos casos una cámara pulpar para los dientes fusionados.

Estos dientes completan su erupción ya fusionados, no quiere decir que se unen de repente ya desde la gestación estos germenes dentarios se fusionan, esta fusión se localiza en la corona de los dientes pero también en la raíces, como lo podemos observar tendrá mayor incidencia o prevalencia en los dentición primaria que en la permanente, el grupo dentario mas afectado es en los incisivos centrales superiores como los que podemos evidenciar en esta imagen, el segundo grupo dentario mas afectado es el canino, es más  común en los dientes superiores que en los inferiores.

Raramente las fusiones afectan a los molares, el tamaño que suele tener es doble debido a la unión que estos tienen, cabe destacar que la etiología se debe a ciertos factores traumático u también por ciertas inflamaciones estas inciden en los folículos de los germenes dentarios, estos hacen que los germenes dentario se unan pero no de una manera adecuada, esta anomalía no representa ningún tipo de riesgo o afecta a los pacientes en funciones básicas como el masticar o etc.

Por último cabe destacar, que se debe establecer diagnóstico diferencial entre geminacion y fusión, puesto que se tiene a confundir estas patologías puesto que ambas son son anomalías de formación, pero la geminacion tiene una gran diferencia puesto que el mismo germen dentario se divide en varios, pero en cambie la fusion son varios que se unen. En niños la fusion tiene un tratamiento restaurador, pero esto dependerá del diagnóstico radiográfico.
