POLST: Physician Orders for Life-Sustaining Treatment. It is basically a medical form where he tells what he wants to be done to him in case he cannot make decisions himself. Like if he wants to be on life-support and stuff like that... The decision wasn't mine, I am just helping my mother in law to place him in the home.
