The device is hooked into the motor cortex region (the region responsible for controlling movement) of her brain. The device registers her brain impulses as she thinks to move the cursor. The small hand movements are all that she has as far as motor control, and are resultant from the thought necessary to control the cursor.

They used another participant that had slightly more motor control in her hands which they had to account for in their studies to ensure it wasn't giving her an advantage over the other participants.

These people are paralyzed but that doesn't mean they are completely static, miniscule movements are still possible.
